Life Assured.

Medical Ceritificate, with Certificate of Identity attached.
These Certificates to be on the printed forms furnished by the Company.

Registrar's Certificate.
The Certificate of the Death from the Registrar of the District in which it occurred.

Proof of Age, if the Age have not been admitted on the Policy.
A Certificate of Birth, or of Baptisn. If neither of these can be procured, an ext-
duly certified, from a Bible or other Family Record, should be obtained,
Certificate of Baptism be furnished which does not contain the date of [B?]
it should accompanied by a Declaration made before a Magistrate by [?]
member of the family, or intimate friend, for which a form will be supplied-
the Company.
