Flying Officer Meredith was captain of a stirling bomber which took off on air operations on the 1st October 1942 to the target of Lubeck in Germany.

The aircraft failed to return to its base and all members of its crew including Flying Officer Meredith were classified as missing. Due time having elapsed Flying Officer Meredith's death was officially presumed to have occurred on the 1st October 1942 as the result of air operations Exhaustive search subsequent to the cessation of hostilities have led to the assumption that the aircraft went down at sea and that Flying Officer Meredith together with the other members of the crew lost their lives at sea.
