Summary: Brooks was drafted 20 Sep 1864 and died 6 Dec 1864 in Louisville of Erysipelas. He was admitted to the hospital 24 Oct 1864. There was a note about how he may have died from Small Pox but it was crossed out. James and Mariah were married 20 January 1860. This was shown by affidavit of witnesses and officiating clergyman. They had one son William H who was born 4 Nov 1861 and died in 1866.

Summary: Mariah, widow of soldier, was 25 years old in December of 1865 when this document was filled out. James died on or about the 7th of February 1863. Illegible cause of death at Park Barracks in Louisville. Mariah stated they were married 20 November (October crossed out and November written above). She has remained a widow since his death. Her home was on wagon road leading from Louisville, KY to West [illegible]. Child William Henry Brooks was born before the marriage of Mariah and James. No other details given for the child James Robert.
